At Home by Silverstone Living offers a life care style contract option to our members. Members will pay a one-time initial membership fee which is based upon your age at the time of enrollment and then pay a predictable monthly service fee.

Working with you and your professional and personal support network, your Care Coordinator:

• Establishes a relationship with you today to better manage your care tomorrow.
• Promotes wellness and preventative care.
• Is available to you by phone, email or text.
• Manages and coordinates your care as needed.
• Assures quality services with a thorough vetting process and continuous oversight of that care.
• As a guide and advocate in long-term care, your Care Coordinator is there for you along the way.

Our program is an option for healthy, active, 62+ adults who want to remain in their homes with a long-term care plan in place for tomorrow. These adults do not want just any plan; they want a plan that ensures a predictable fee and personalized coordination and management of that care. They also prefer to age in place, remaining in the home they love regardless of future health changes.
